FOSTER PARENT OF THE YEAR AWARD
This award is to recognize a Foster Parent or Foster Family who have demonstrated exceptional accomplishments in the field of fostering. (not necessarily over a long period of time)

CRITERIA:
- Foster Parent must be in good standing with FPSO;
- Cover sheet must be attached including the name, address, and telephone number of the Foster Parent being nominated, and the name of the FPA/FFA or Agency making the nomination;
- Maximum of two (2) typewritten pages outlining the background of the nominee must also be submitted; and
- Only one (1) submission allowed by FPA/FFA or Agency.

LIFETIME ACHIEVEMENT AWARD
This award recognizes a current Foster Parent or Foster Family who has demonstrated continual lifetime excellence in the field of fostering as well as within the community. This nomination can be made by an agency, FPA/FFA.
CRITERIA:
- Foster Parent(s) must be in good standing with FPSO;
- Cover sheet must be attached and include the name, address, and telephone number of the Foster Parent(s) being nominated, and the name of the FPA/FFA or Agency making the nomination.
- Maximum of two (2) typewritten pages outlining the background of the nominee(s) must also be submitted;
- Only one (1) submission allowed by FPA/FFA or Agency
